Student Financial Assistance
The Office of Student Financial Assistance assists students and their families in meeting their college expenses. Many federal, state, and private sources of assistance are available. While many financial aid awards are determined by financial need, the College also awards loans not determined by need and a number of scholarships based on merit and achievement.

2011 - 2012 Forms      

2011-2012 Financial Aid Application for Summer
The Financial Aid Application for Summer is used to request financial aid funds for the Summer I and Summer II sessions. Students should be sure to carefully read all information listed on the application.
The deadline for the application is April 13, 2012.

Consortium Agreement
This packet contains the instructions and form necessary to complete the consortium agreement.

Revision Request form 2011-2012
A Revision Request form is used to make cancellation or reductions to an award that has been offered to a student from the Office of Student Financial Assistance. This form is not a means of applying for financial assistance, a Free Application for Federal Student Aid must be submitted prior to a Revision Request form.

Independent Verification Worksheet 2011-2012
An Independent student is defined as a student who is or will be 24 years of age on or before December 31 of the academic year; is working toward a masters or doctorate program; married; has children or legal dependents other than spouse; orphan or ward of the court until the age of 18; veteran of the U.S. armed forces. Independent student's selected for verification are required to complete a verification worksheet. The worksheet and a signed copy of student and spouses Federal tax return should be submitted to the Financial Aid office before the students eligibility can be evaluated.

Dependent Verification Worksheet 2011-2012

A Dependent student is defined as a student who is under 24 years of age as of December 31 of the academic year; has not completed a Bachelor degree program; not married; does not have children or legal dependents; not an orphan or ward of the court until the age of 18; not veteran of the U.S. armed forces. Dependent student's selected for verification are required to complete a verification worksheet. The worksheet and a signed copy of student and parents Federal tax return should be submitted to the Financial Aid office before the students eligibility can be evaluated.

Academic Appeal

Students who have not maintained Standards of Satisfactory Academic Progress (SAP) will be required to appeal to reinstate financial aid eligibility. The appeal should be TYPE written, include the reason for the appeal, steps that will be taken to insure academic success and any supporting documentation.

Maximum Timeframe Appeal 2011-2012

A student who has attempted more than 150% of the credits needed to complete their program of study will be required to request an Academic Extension to maintain financial aid eligibility. The extension request should be completed with an Academic Advisor and the student will be limited to the classes approved by the Advisor to complete the program.

2011-2012 Homeless Independent Request
When you are over the age of 21, and you indicated that you were homeless or at risk of being homeless. A student is considered homeless if they lack fixed, regular, and adequate housing. This is broader than just living “on the street.”

2011-2012 Request for Independent Status
You may submit this form to determine the dependency status for students who have documented, extreme, and extenuating circumstances where it would be unreasonable or impossible to provide parent(s) information on the FAFSA.

2011-2012 Request For Review Of Special Circumstances
Request for Special Circumstances is available to assists students who have a sudden reduction of employment, income or benefits. Request for Special Circumstances (RSC) will NOT be processed until after the 1st disbursement period of each term.  If your RSC is approved, revised eligibility will replace any existing financial aid award(s).

You must use this form to update information with the Admissions and Records Office

2011-2012 Student Information Release Authorization
Under the Federal Family Educational Rights and Privacy Act (FERPA), Paradise Valley Community College Office of Student Financial Assistance is required to protect student information.  If a student would like to request confidential information be released to a third party, the student must submit this form to the Office of Student Financial Assistance.

2011-2012 Federal Loan Discharged Due to Disability
Our office has received information from the National Student Loan Data System (NSLDS) indicating that you have had one or more prior student loans discharged due to total and permanent disability. In order for our office to continue processing your financial aid, we need you to clarify whether or not you want additional loan consideration.

2011-2012 FAFSA Parental Data Override Form
This form documents the lack of parental data on the Free Application for Federal Student Aid (FAFSA) in order for a student to qualify for Federal Unsubsidized Stafford Loans. Submit completed form with the required documentation to the Financial Aid Office.

2011-12 Household and Untaxed Income Independent Worksheet

An independent student who completed the FAFSA by using the IRS Data Retrieval process and is selected for the verification process are required to complete a Household and Untaxed Income Independent Worksheet.  The student must complete this worksheet and return it to the Office of Student Financial Assistance before the student’s eligibility can be evaluated.

A dependent student who completed the FAFSA by using the IRS Data Retrieval process and is selected for the verification process are required to complete a Household and Untaxed Income Dependent Worksheet.  The student and their student’s parent must complete this worksheet and return it to the Office of Student Financial Assistance before the student’s eligibility can be evaluated.
