Arizona state law requires any person who participates in a teacher preparation program to obtain an “Identity Verified Print” (IVP) Fingerprint Clearance Card. Arizona school districts require that college students be fingerprinted prior to participating in field experiences at their schools. When requesting an application packet please make sure you specify that you need the “IVP” application packet.
What is the fee to obtain a Fingerprint Clearance Card?
The DPS processing fee is $69.
What forms of payment are accepted?
DPS accepts Cashier’s Checks or Money Orders.
All forms of payment should be made payable to the AZ Department of Public Safety.
Where can I obtain an application packet for a Fingerprint Clearance Card?
Students may go to the UPS Store, located at 3317 E Bell Rd in Phoenix, to obtain an application packet and to have their fingerprints rolled. Students must bring their $69 cashier’s check or money order made out to DPS to cover the processing fee.
The cost for rolling prints will be an additional $10 for students who bring in their PVCC student ID.
