PVCC Wellness: Wellness Watch

Any PVCC employee may nominate an employee for the Employee Wellness Role Model of the Month by completing a nomination form. Employees may nominate themselves. On the form, the nominator will provide a brief description of why they think the nominee deserves this recognition.
Nominations are accepted from the 16th of one month to the 15th of the next month – see below timeline.
All nominations will be collected and submitted to the members of the Wellness Committee.
The Wellness Committee will vote on the nominations and decide the winner of the month.
In the case of a tie, both employees will share the award.
An employee can only win the award once in a 12 month period.
Winners of the award will be notified. If the winner accepts the award, their picture will be posted on the winners page along with a write up about why they were nominated for the award. Fliers will also be distributed to the departments on campus.
Winners of the award have the option to decline the award. In that case, the runner up will advance to the winner position.
Winners will receive a fabulous prize at the closing ceremonies of Wellness Month.

Sample Timeline for the April Employee Wellness Role Model
March 16 – April 15	Nominations are accepted. All nominations must be submitted via the website. All nominations are collected by a member of the Wellness Committee and a 2nd party.
April 16 or 17th	Members of the Wellness Committee will be e-mailed a listing of all nominees and the descriptions of why these employees were nominated for the Wellness Role Model of the Month. Wellness Committee Members will be allowed to place one vote for the Wellness Role Model of the Month. Only nominations that have been submitted via the website will be considered. The nominee with the most votes from the Wellness Committee will receive the monthly award.
April 25	Last day for Wellness Committee Members to vote.
April 26	Voting is closed and the results will be tabulated.
April 26 or 27th	A member of the Wellness Committee will contact the winner and asks for photos and a description of the role the wellness plays in their life and how it affects their well-being.
May 1st	The winner is announced to the campus via the wellness website, campus-wide e-mail, and other fliers.
